No legal authority was there to witness his name change, therefore he was still Darth Vader at the time of his death on galactic records.
He also suffered serious injuries at the time of his declaration, I would argue that he was not mentally competent to be aware of his actions. Thus, any name change was invalid.

I've probably posted this like 17 times but we get like 6-8 weeks of what i consider unbearable heat here (~95 degrees but no humidity) and i don't know how people live in saunas for half the year.
