If you buy the game, what you're paying for is the campaign. (And likely extras for multiplayer like skins, etc.)

I’m sure it varied from game to game or platform. I remember getting kicked from a lobby a couple times for not having a map pack.
I don’t recall it happening with CoD4, might’ve been one of the Treyarch games or MW2.
