Well there isn't if it's for actual surgery or medical reasons, which is why you can get permission as the article says. But clearly it wasn't quite, when it was refused twice by Uefa and CAS, before he went on and did it anyway. Apparently it can be used to mask other doping.

It never says he got permission anywhere that I can see, for breaking the 50 millilitre infusion limit per six-hour rule. He was feeling ill and went and broke the rules, that a professional footballer certainly of his status, should be well aware of. Sevilla then tried to get a retroactive (why would they have to... if he already got it) exemption, which was not granted... Both by Uefa and CAS.
