You can tell the defense exactly what you are going to run on offense before you run it and it won't make a lick of difference. It's down to recognition, reaction, and execution.
Coaches with the best oob plays are the ones that practice them the most. Design a play with several options at many points as the play develops and drill it into the players until they can execute it while sleepwalking.
Execution (and shot making) is what makes plays work. Not "surprising" the defense. They already know what your team and players want to do and they have a scheme to stop it. They know that if you try to do something you haven't done before, odds are you gonna fuck up. So they ain't worried about that.
They know they have to execute their plan better than you do yours.

They have. And it's been obvious at times he's been going for his triple double stats.
But, w/e.
I mean, it's literally why people bother to watch the Thunder. It does cheapen the "average a triple double" MVP defense. But most absurd stats like that have had padding in them somewhere. Stocktons assist count home vs away is the classic. But i'm sure that was the same for many players.

This injury will require arthroscopic surgery (his fourth knee surgery) at the worst possible time as he's set to hit free agency this summer. He's slow laterally and he has the some of the worst defensive awareness I've ever seen. He has the peripheral vision of a Metal Gear solid enemy.

It's funny he improved vastly in the midrange range and made better use of fakes and floaters, but the #1 thing that killed him is a complete lack of BBall IQ. Can't read defenses well, can't run anything more than a PnR Offense with a few wrinkles. Literally didn't recognize the O when Knicks ran similar plays that he ran with the Bulls, and for the love of God he can't even fake being interested in setting up other players --- even though he's the PG. There's no reason he can't be a homeless man's John Wall, picking when to attack and looking to set up teammates early; he just has no awareness of when (and sometimes why) to do any of those things.
He won't be as good as a Lou Williams, but he should be real solid.
