If you can't get your players to stay switched on just after you've scored after this long, and after it's been exploited by the opposition so many times...it's on the manager.
It's also more liable to happen, probabilistically, when you're conceding a shot every 3 minutes (which is a feature and not a bug).

What this game exposed is we have too many players who are frequently inconsistent, and over reliance on our youth to pull us out of danger.
Man like Rash and Bruno doing nothing add to that Mctom and you've a team with too many inconsistent performances to get you through a season.
These lot don't look or perform any better with a new manager. Kneejerk "sack the manager" anytime there's a poor result or performance is tiresome.
If as players you have no idea of the things you can and cannot do on a pitch, you will never achieve anything. So many times this Manu team is stretched all over the pitch, that every opponent without even trying finds space literally everywhere.
If that's team set up or players with no brains, for me mostly lack basic football IQ, you need a big change in that squad. (or at least maybe a starting 11 fit for few months for things to improve) Don't think another new manager with same players will change a lot.
Appoint a good DOF, find a style and fitting manager, clean up the squad and trust that process. Maybe they will finally do it this time.

Barring any more injures only Luke Shaw is absent from his ideal starting XI.
If we end the season playing as shit as this and being so open, where teams like fucking Brentford are playing you off the park, I think it's only fair the criticism goes past just players not being good enough. He's setting the team up terribly and had the excuse of injuries up to now.
There has to be a clear improvement in how we play from now or he should be gone imo.
